Care Coordination

SCSN can partner with medical provider and other in-home or other living facilities to make sure clients are receiving quality care.  Our staff can:

• attend plan of care staffings/meetings

• regular visitation while hospitalized in a facility 

• verification of scheduled services from other providers

These personalized services can be scheduled seven days a week.
